Each case is certified for 2 hours of Category 1 CME credit. There are 3 steps you must take if you wish to earn credit.

1. You need to provide written answers to the discussion questions that appear in the case material.
2. At the end of the course, please complete the evaluation and survey.
3. Print and complete the CME registration form and survey which will appear at the last page of the course. Mail them together with a check for $ 30.00, payable to "CHP" to:
